Marriage or Divorce Name Change

After a name change due to mar­riage or divorce, a new mem­ber­ship appli­ca­tion must be com­plet­ed by the respon­si­ble par­ty.

If a mem­ber needs to request a name change for a rea­son oth­er than divorce or mar­riage, you will need to call us or come in the office.
